Who is eligible to attend the EverJoy Empowerment Hub?

Adults with physical disabilities, developmental challenges, mental health needs, or aging-related conditions in Dane County are eligible. We serve both ambulatory and non-ambulatory individuals.

What is the enrollment process and timeline?

After referral submission, enrollment typically takes 3–10 business days. This includes eligibility verification, intake assessment, and care plan development to ensure the best fit for the participant.

Does EverJoy offer transportation?

Yes—we help coordinate transportation through local providers and IRIS/Family Care transportation services.

What payment sources are accepted?

We accept Medicaid (through Family Care and IRIS), Managed Care Organizations, and private pay.

How does the EverJoy Empowerment Hub support participant independence?

We focus on person-centered care that promotes autonomy, skill development, and community integration through individualized support plans

What are your hours of operation?

Our center operates Monday through Friday, 8:00 AM to 4:30 PM, excluding holidays.
